Dear West London Mental Health NHS Trust,
Can I please have information on SUI procedures in place on Kingfisher Ward at WLMHT Lakeside Unit 2004-2010
I need to access information on:
What specific procedures are in place regarding reporting unexpected death and a)under what circumstances the police are called b)what process is used for this purpose?

Thank you for clarifying the situation and I take it the "Additional request"will have a 20 day time limit too.
Being mindful of Christmas and New Year please be advised I am prepared to wait until FOI Office reopens in January.

Dear Mrs Speers,
Please accept our sincere apologies for the lengthy time it has taken us
to respond to your request asking to see the 2005 Version of Death of a
Patient Policy. This is due to an extensive search regarding this matter.
We can confirm that D6 Death of a Patient Policy was first published in
2006. The previous two documents I1 Incident Reporting Policy and I8
Incident Reporting and Management Policy (attached again for your
reference) were the documents in place during 2005.
We hope this has cleared this matter up for you. In any event, please do
not hesitate to contact us again if have any further questions.

Thank you for recent communication and I dont appear to have the quoted "documents attached". Could you resend please.....I1 Incident Reporting Policy and I8 Incident Reporting and Management Policy..and confirm when publication of these policies ceased?
Could you also clarify:
a) if the previous two published documents were in place of death of a patient policy D6?
b) Policy D6- "Death of a Patient policy" which you kindly informed me was first published in
2006. Please advise of the time lapse between both publications.
