Insolvency Practitioners Tribunal
The Insolvency Practitioners Tribunal (IPT) was a specialist body which heard appeals from people who have had their accreditation as an insolvency practitioner removed or refused by the Secretary of State.
Under the Deregulation Act 2015  the Secretary of State ceased to authorise insolvency practitioners directly, and the tribunal closed shortly thereafter.
This authority no longer exists, so you cannot make a request to it.
