Dear Sir or Madam,
Background information to my Freedom of Information request.
Following an on-line petition about the Local Government Ombudsman
a response was eventually sent to all people who signed the
petition by the Prime Minister's Office. I am one of those people.
As a result of a material inaccuracy I submitted a Freedom of
Information request to the Prime Minister's Office. They have since
informed me that your office supplied the flawed information their
response was based on.
Therefore, under the Freedom of Information Act I would like a copy
of any and all documentation your office used to create the flawed
response to the Prime Minister's Office together with any
documentation supplied to your office by the Local Government
Ombudsman regarding this matter.
Full background details can be found here,

Dear Juliet Tomlin,
There appears to be some confusion. After I submitted my request
for an internal review I received the information I sought.
Therefore, a internal review is no longer necessary.
