Dear Sir or Madam,
I understand that the SHA and PCT have joint plans in place with
the Dept of Health to achieve at least a 20% reduction in the death
rate from 'injury and undetermined death' by 2010.
The Dept of Health have kindly confirmed the withdrawal of suicide
indicators, which as I understand has already happened at PCT level
and also have confirmed full
withdrawal of indicators, intended nationally from 2010/11.
Please can you provide information on these joint 'plans' and how
does Oxfordshire PCT, fit into these plans?
Can OPCT provide information on how much progress has been made
towards the 2010 target?
If PCT's are "expected to support SHA's in delivery on their agreed
priorities" how is this to be achieved now that PCT indicators have
been withdrawn?
Yours faithfully,
D. Speers

Dear D.Speers
Information Request
Further to your email of 14 October 2009 in which you requested
information under the Freedom of Information Act 2000 regarding suicide
prevention and reduction in rates, we are pleased to be able to provide
you with the information you requested.
I understand that the SHA and PCT have joint plans in place with the Dept
of Health to achieve at least a 20% reduction in the death rate from
'injury and undetermined death' by 2010.
The Dept of Health have kindly confirmed the withdrawal of suicide
indicators, which as I understand has already happened at PCT level
and also have confirmed full withdrawal of indicators, intended nationally
from 2010/11.
Please can you provide information on these joint 'plans' and how does
Oxfordshire PCT, fit into these plans?
Can OPCT provide information on how much progress has been made towards
the 2010 target?
If PCT's are "expected to support SHA's in delivery on their agreed
priorities" how is this to be achieved now that PCT indicators have been
withdrawn?
Oxfordshire PCT has been working towards the target of a 20% reduction in
the death rate from suicide and undetermined injury by at least a fifth by
the year 2010 and is currently on track to achieve this target.
Progress towards this target includes:
. A systematic review of every suicide and undetermined injury by
provider organisations including the Oxfordshire & Buckinghamshire Mental
Health Trust.
. A two yearly audit of suicides and undetermined injury. The last
audit took place in 2007 and the next audit is currently being undertaken.
. Developing an action plan for a more strategic approach to
suicide audit and prevention
The South Central Strategic Health Authority requires Oxfordshire PCT to
conduct a suicide audit as part of the `Autumn Assessment' process. In
addition Oxfordshire PCT is currently reviewing its processes of suicide
audit and prevention strategies, in order to make improvements.
We do hope you find this information useful, but if you have any further
queries then please contact us at the above address.
If you are unhappy with the service you have received in relation to your
request and wish to make a complaint or request a review of our decision,
you should write to the: Deputy Director, Head of Quality and Clinical
Standards, Oxfordshire PCT, Jubilee House, 5510 John Smith Drive, Oxford
Business Park South, Oxford, OX2 2LH.
If you are not content with the outcome your complaint, you may apply
directly to the Information Commissioner for a decision. Generally, the
ICO cannot make a decision unless you have exhausted the complaints
procedure.
The Information Commissioner can be contacted at: The Information
Commissioner's Office, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9

Dear D.Speers
Information Request
We are treating your email of 3 November 2009 as a new request for

new questions on our Freedom of Information database and make sure we keep

are different from the questions in your original request for information
so it will take longer to respond to them as new questions than it would
take to provide clarification for questions.

Dear D.Speers
Information Request
Further to your email of 03 November 2009 (which we agreed to treat as a

Provider organisations including Oxfordshire and Buckinghamshire Mental
Health Trust systematically review each death from suicide and
undetermined injury. This data is compiled by Oxfordshire PCT's Clinical
Governance Department. Progress towards reduction of deaths from suicides
and undetermined injury is measured via the two yearly suicide audit.
The two yearly audit of suicides and undetermined injury is currently
being undertaken. This is required by the South Central Strategic Health
Authority as part of the 'Autumn Assessment'. This audit is led by Public
Health in conjunction with other departments within the PCT and is due to
be completed by the third quarter of 2009-10.
Oxfordshire PCT is currently reviewing its processes of suicide audit and
prevention strategies, in order to make improvements. An action plan for
a more strategic approach to suicide audit and prevention is being
developed. This action plan will be completed by the first quarter of
2010-11.
Oxfordshire County Council's Overview and Scrutiny Committee have not been
involved in this review as they have not had any concerns. Should this
change, Oxfordshire PCT would cooperate fully.
We do hope you find this information useful, but if you have any further
queries then please contact us at the above address.
