Dear Sir or Madam,
May i please request the number of suicides covering England over the last 5 years, those in hospitals, Mental Health units/hospitals, other facilities ie care homes, in the community etc and the authorities covering these. Would you also confirm if these people were under any care/supervision. May i also request any coroners reports and outcomes of said reports and if any patience/people were sent from another authority to another for care when the suicide took place. Would you acknowledge if any legal proceedings were brought because of these and the outcomes of the said proceedings
