Dear House of Commons,

Please treat this as a request under the Freedom of Information Act
2000.

May I request a (blank) copy of the form, preferably
electronically, issued to MPs' researchers, Select Committee
advisers and so on, regarding the issue of Parliamentary passes.

It is the one which asks questions such as, "Have you ever been
involved in terrorism? Espionage? Sabotage? Tick as applicable." It
also queries whether the applicant has ever attempted to overthrow
Parliamentary democracy.
