Reported deaths/severe harm incidents

Dear Sir or Madam,

1.In the 08/09 financial year on how many reports made to the
National Patient Safety Agency (NPSA), on (i) deaths (ii) severe
harm came from (i) NHS Trusts (ii) with Foundation Trust status
(iii) with Foundation Trust equivilant. Please give the breakdown
between Acute Trusts and Mental Health Trusts

Of these how many reports to the National Patient Safety
Agency (NPSA) where the incident was (i)medication,and (ii)
clinical assessment?

3. For all the incidents where the degree of harm was recorded
as‘death’ please give the type of incident eg. Patient
accident,treatment procedure etc.

4.For all the incidents where the degree of harm was recorded
as‘severe harm’ please give the type of incident eg. Patient
accident, treatment procedure etc.

The recent investigation report into Patient Safety at West London Mental Health Trust www.cqc.org.uk and the Incident at Broadmoor Hospital Investigation Report (which WLMHT also manage) has raised serious concerns regarding the implementation of policy and correct investigation being carried out in the event of a 'serious untoward incident' or adverse incident (one and the same I believe!)
I am seeking answers on if deaths are being reported as they should be and maybe this will given an indication as to how many vulnerable people are trying to survive in the community. If left unsupported by the services then this put them and us all at risk....its the managment of services that needs addressing.....not cuts to front line staff!

Information reviews. I have spoken to senior staff on the
telephone, with the promise of further contact and as this has yet
to happen, I now need more clarity of response please.

Dear Gay Kennedy,
Many thanks to Dr Cleary and you for your comprehensive response. I
appreciate your time and your phone call last week. As you will
have deduced I am left feeling deeply disturbed by the numbers of
SUI which seem to be being misreported and I wonder how the NSPA
can tackle the challenge of a "confidential and anonoymised
reporting system?" Could something confidential on the lines of
SUSARS be viabale? Where each hospital and GP surgery had feedback
forms for collection by LINks........for confidence and security,
in lockable type ballot boxes?
I note your comments on FTe and refs to the three trusts where
application for FTe is pending.

Dear Gay Kennedy,
I note that you have passed on my comments to Dr Cleary who has not
made any further contact.

In the light of the recent reports into failures at
Mid-Staffordshire, Basildon and Thurrock Hospital, failures
identified by HM Deputy Coroner,at the maternity unit at Milton
Keynes Hospital and now failure by Sussex Partnership MH Trust to
publish the findings of the attempted murder of Lucy Yates by
Samuel Reid-Wentworth (who was under the duty of care of SPT.)

The resignation of Baroness Young from the Care Quality Commission
Chair, the responses to and the results of The Parliamentary Health
Select Committee Report into Patient Safety were commented on by
Lord Patel as National Patient Safety Agency Chair. But as Chair of
the Mental Health Act Commission Lord Patel was very aware of the
tragic death of my son as an inpatient West London Mental Health
Trust, who were the subject of two Investigations this year ...one
into patient safety and the other into 'an incident' at Broadmoor
Hospital.....where Samuel Reid-Wentworth has been sent for support.
When is there going to be some joined-up thinking on these issues!
