Thank you for your reply.

I find it hard to believe that you do not keep an electronic copy
of the document requested unless it was created on either a
typewriter or created by hand.

Please therefore confirm:

i) In what format do you hold the Pupil questionnaire? (Hand
written note, MS Word Document, PDF etc etc)

ii) How and when was the document created?

Perhaps you could scan the document and attach it to your reply
