Dear Sir or Madam,
Kindly supply me with:
1 - The total number of individual members of public (if any) that
have reported, with DOCUMENTED EVIDENCE (as in this download

individuals that made up the HEATH GOVERNMENT and SUBSEQUENT

2 - What positive action, if any, by yourselves was taken on
receipt of the said documented proof?
3 - If not taken seriously and dismissed:
a) how far up the chain of command the reported crime go until
enquiries were terminated?
b) what is the name of the public servant that made the decision?
c) what was the reason for the decision?
d) does that public servant still hold his/her position of
authority?
4 - If more than one report has been made to yourselves, was the
same public servant responsible for the decision not to submit the
documented evidence for the prosecution of the criminals.
Thank you.
Yours faithfully,
Keith

Dear Mr Keith
Freedom of Information Request 2009/
Thank you for your recent request made under the Freedom of Information
Act 2000 which was received in this office on the 14th May 2009.
Section 8 (1) of the FOIA sets out the requirements of a valid request
form information and says that a request must, amongst other things
“state the name of the applicant and an address for
correspondence”.
It also states under Section 8 (1) (b) of the FOIA indicates that the
real name of the applicant should be used when requesting information
and not any other name, for example a pseudonym.
The definition of “applicant” in section 84 of the FOIA adds weight
to this as the phrase in section 8 (1) (b) should be read as “the name
of the person making a request”. This also suggests that the use of a
false or fictitious name is not acceptable. Therefore when Gwent Police
receives a request from a person using an obvious pseudonym, we are
under no obligation to comply with the request; nor would it fall within
jurisdiction of the Information Commissioner.
Gwent Police do however have a duty to advise and assist under section
16 of the FOIA. On this occasion to allow us to proceed with this
request we require your real name to be provided either via email or in
writing. Until this information is received we are unable to proceed any
further.
Yours sincerely
