Dear Sir or Madam,

I should be obliged if you would give details on all applications
for NHS Continuing Care Funding that were supported by Multi
Disciplinary Team Decision Support Tool Recommendations.

Please supply detail on Appeals if the original application was not
successful.

Please supply details on all complaints and ongoing legal acctions
in relation to PCT applications for funding to Panels.

Please supply detail on Panel remuneration arrangements and legal
insurance policies paid for by the public purse.

I would be grateful if you would supply as much detail as possible
on all adjudications.
