Dear Sir or Madam,

Are medical records put on computers by people following contracts,
remits and criteria, in favour of medical judgement , as my GPs, in
the past, have told me they are rationing and don't have to use
their ethical code. Is that because they are not fully qualified ?
In my case specialist's letters and tests were not put on computer
which could have prevented cardiac arrest 2 years later?! Is this a
serious issue or not ?

Are the accountants directing the admin directing the Doctors they
buy in ? LAs now appear to be directing PCTs by holding the funds
but are not contactable and are blamed if you wish to communicate
raising health and safety issues.
If you have chronic conditions you are passed from pillar to post
with no objective research. in tier i/2 hospitals with no
choice.Surely this is not economic?

Dear Sir or Madam,

Are you delaying on purpose ? As I have informed you and many
health organizations over the years of this procedure causing
problems. Surely this is an urgent health and safety issue but all
call centres including Healthcare Commission deliberately leave it
limbo ???!!How can we prevent further delay? Maybe an internal
review
since nobody appears to be able to deal with.So I request an
immediate internal review .

I suggest you read the guidance on this site and elsewhere about how the FoI process works - the authority is entitled to take 20 days to answer, and in practice often answers earlier. You can't request an internal review until you've had a substantive answer that they can review.

Where is the guidance as most organizations try to go back to beginning each time so never reach the end so don't have to show they have no discretion even the charities .What I used to sort out in 5 min[ very ethical] never gets sorted getting ripped off by solicitors and those with responsibility for assisting in the process. Are you taking sides? I feel totally alienated my dyspraxia [ location /sequencing] is being used against me so can't raise issues. You are using same written not oral system causing me the problems to silence me highlighting problems for everyone which is a shame as the basic ideas behind your sites are excellent it is following admin and process causing the bother as usual .Otherwise why am I not finding any of the excellent methods effective.

How does that resolve the problem . I am not asking for recorded information because if it is maladministrated noone does anything anyway . So what is the point?

You're not getting satisfaction from the FoI system because you're using it for things and in ways it wasn't designed for. It's for getting access to authority records. It's not appropriate to attempt to engage the FoI officers dealing with your request in long discussions about the policies of the authority, or about any personal grievance you may have with the authority. You should explore the authority's complaints procedure for that. If you have communication difficulties then the authority should make their procedure accessible to you in other ways.

Do you think I haven't & its been deliberately destroyed by the Royal Courts of Justice after paying £2000 to a dodgy lawyer and nobody wants to know. . Its people like you giving others so much stress it causes cardiacs .What have you done to help ??!!If I was treated fairly properly I would not be writing a word this has only happened since Thatchers Cut"s and suspect Research methods.Obviously you are benefiting from this corruption to be so mean.

None of which has anything to do with the FoI process and the DH. You really would benefit from taking a breath and concentrating your efforts in the right places and leaving the FoI process for those who have a valid use for it.

And no, to my knowledge I've never benefitted from the corruption due to Mrs Thatcher's suspect research methods. Whatever that may mean.

The FOI Act entitles you to ask for recorded information only. If you write and ask for the department's opinion they will rightly not treat it as an FOI request but will treat it as departmental correspondence and provide you with a reply. The Department of Health probably has a complaints procedure which you can use, but this won't be an internal review as part of FOIA. You may find this irritating, but best to know how the system works.

YOu should also be aware that your annotations will not be seen by the Authority in question. This is not as you may think a 'web forum' it's not a real time way to raise problems but a nifty way of allowing people to send FOI request to public authorities.

Therefore many of teh comments you see (the ones in a smaller font) will only be seen by people who are reading your attempts at righting what yu belive to be many wrongs. Unfortunately this may be the wrong way of doing so
