Dear Sir or Madam,

In reference to your document titled "GENERIC FRAMEWORK
FOR MASS VACCINATION" (available at
http://www.bolton.nhs.uk/Library/your_he...)
I would like to seek further clarification of your plans in respect
of an H1N1 pandemic emergency.

In regard to the the Example Invitation Letter (page 29) for a
vaccination appointment:

1. What further advice will your organisation give to persons who
intend to decline to receive the vaccination for reasons other than
allergies (e.g. personal choice - cultural/religious/lifestyle
reasons)? In particular would this advice cover other methods to
reduce the risk of contacting flu?

2. How will such advice be issued and will it be issued in such a
way as to reduce the numbers of such persons calling helpline
numbers if there is high demand placed on call centres (e.g. in
leaflet format included with the invitation or online)?

3. Will a fee be levied for vaccination at a later date if
individuals fail to turn up for the designated appointment without
cancelling (as is sometimes the case with e.g. missed dental
appointments)?

4. What further communication will be sent to individuals who fail
to attend either the scheduled appointment or one of the later
appointments mentioned? For example will alternative arrangements
be made available or will they be able to arrange an appointment
for vaccination at a GP surgery or health centre at a time of their
convenience or will they be denied vaccination?
