I have just opened a copy of the cd. I had difficulty finding Appendix 4 initially, but after closing down and going back in all the information was there on page 191-234.
I wonder if you could try again. I will have to print, scan and email information otherwise.

nothing comes up on any of the appendices. Since the file size is
only 823kb, it seems too small to contain this info!
