I would respond as follows
a) this information is not fully available at this authority as records
for the last ten years are not all available, the Council having a
practice of discarding non essential records older than six years. The
information may be available at the office of the Local Government
Ombudsman. I can advise from personal knowledge that this authority has
had no full findings of maladministration leading to an injustice since
its inception in 1974
b) To my knowledge, none
c) To my knowledge, none

A full finding is a a finding of maladministration leading to an injustice
