We have now considered your request for information and a copy of the

Please note that the name of the company has not been provided where it
refers to an individual. This information is exempt under section 40 of
the Act and will not be released.

Section 40(2) of the Freedom of Information Act 2000 allows us to withhold
information if it relates to someone other than the data subject and where
disclosing that information would contravene the principles of the Data
Protection Act 1998.
