Dear Sir or Madam,
During the last five years how many times has the Local Government
Ombudsman (as a result of investigating a complaint against Isles
of Scilly Council) brought to the attention of the council the fact
that a member of Isles of Scilly staff had misled them, lied to
them or done anything else to hinder their investigation. If they
have what disciplinary action was taken against the individual(s)
concerned by the council.

I am sorry for the delay. I have just returned from an extended holiday and will deal with your request as a matter of urgency. We are a small Island 1.75 x 1.5 miles long with a population of under 2,000. Unfortunately we do not have a department that deals solely with FOI requests due to a limited population.
