Dear Sir or Madam,
Please provide details as to how many Local Government Ombudsman
(LGO) investigations and also complaints submitted have over the
last ten years involved a death due to alleged maladministration by
a Council.
Further detail by year and Council what the LGO's finding's were
(i.e. MI, M, Discretion, etc.).
Please finally identify what monitoring the LGO undertakes to
ensure that its recommendations and remedy's are met by any
Council's against which it makes a finding that involve a Council
having been identified as having caused a death through its own
actions or those of its contractors to ensure compliance, and if
the LGO undertakes any monitoring to ensure that it is able to
correct maladministration of this sort by Council's.

the LGO is known to keep statistics on issues where a formal report
is not issued. I wish to know how many complaints to the LGO that
did not result in a finding of MI or LS were based at least in part
on a death.. (i.e. Ombudsman's discretion, M, Premature, No M etc).
Please additionally identify by name the LGO responsible.

I can accept that you don't have any information. However, I
believe that you should hold the information... so that the
boundaries of the Ombudsman's discretion can properly be
identified. It is understood that Ofsted (the LGO London's friend
is now Head of Ofsted) face similar problems.. particularly in
relation to Haringey.

Investigations by the Ombudsman quite frequently involve deaths caused by Council's as a result of maladministration - and the Council's frequently reject both the Ombudsman's remedy and recommendations.
