Dear HPCT

I would like to some requests under the FOI Act.

For the purposes of the Act, the date of your receipt of this
request will be 13th January.

Back in 2008, I made an FOI request to Hampshire PCT requesting
minutes of any meetings held regarding implementation of the SCR.
In your FOI response to me dated 30th July 2008, you stated the
following:

"Hampshire PCT has no plans as yet to implement SCR, and therefore
no project board has been set up."

"There was a plan to set up a CRS Project Board, however this has
been temporarily shelved due to the collapse of the Fujitsu
contract."

"At the present time there are no meetings planned."

EHI has today reported on the further rollout of the Summary Care
Record:

In that article, HPCT is identified as a potential fast follower,
that is defined as ready to start sending SCR material to patients
from (at least) two (iSoft) GP practices before 1 April 2009.

My requests are as follows:

• Please would you send me the minutes (draft or approved) of any
HPCT meeting (e.g. Project Board, Project Implementation team)
regarding the Summary Care Record/NCRS/Healthspace rollout created
or updated between 1st August 2008 and 31st December 2008.

• Please would you send me any HPCT SCR/NCRS/Healthspace Highlight
Reports (draft or approved) created or updated between 1st August
2008 and 31st December 2008.

• Please would you send me any HPCT SCR/NCRS/Healthspace Issue Logs
and Risk Logs (draft or approved) created or updated between 1st
August 2008 and 31st December 2008.

• Please inform me as to what meetings now take place within HPCT
on a regular basis regarding the forthcoming or potential SCR
Rollout, and what updating documents (such as logs or
board/implementation team updates) are produced on a regular basis.

Please could you also send me:

• The latest version of the patient letter that you are planning to
send out to residents about the SCR.

• The latest version of the SCR opt out form that patients will be
using.

• Please could you identify the two or more GP practices that you
have identified as being suitable to initiate any fast follower
rollout.

As per Section 4a of Part 1 of the FOI Act
(

) I would like the information in question held at the time when my
request is received (draft or otherwise), except that account may
be taken of any amendment or deletion made between now and the
latest time by which the information is to be communicated to me,
being an amendment or deletion that would have been made regardless
of the receipt of my request.

I would be grateful if you would be kind enough to send me the
requested information promptly and in any event not later than the
twentieth working day following the date of receipt of my request -
that is by then end of the working day of 10th February.

If my request is denied in whole or in part, or specific items
within the responses are withheld from disclosure, then you must
justify all deletions by reference to specific exemptions of the
act, as per Section 17 of the Act

). Where you rely on a qualified exemption to withhold disclosure,
you are obliged to consider the public interest in your decision
and the refusal notice must explain not only which exemption
applies and why, but also the public interest arguments addressed
in reaching the decision.

I wish to receive the information by email; I believe such a
request would be reasonable in these circumstances. As per Section
11 of Part 1 of the FOI Act (

) I would be grateful if you would "so far as reasonably
practicable give effect to that preference"; if you determine that
it is not reasonably practicable to comply with this preference
then you are required by law to notify me of the reasons for your
determination.

The “return address” (to which the information that I have
requested be should be sent ) is the email address that this
particular request originates from (i.e. ending in
'@whatdotheyknow.com').

I would be grateful if you would kindly acknowledge receipt of this
request as recommended by the ICO (“It would be good practice to
acknowledge receipt of requests and to refer to the 20 working day
time limit, so that applicants know their request is being dealt
with”).

I am writing to confirm that Hampshire Primary Care Trust (PCT) has now
completed the search for the information you requested on 13 January
2008 under the Freedom of Information Act, where you asked for
information relating to the forthcoming roll-out of SCR.

Please see Hampshire PCT's response to your questions below:

Hampshire PCT completed the SCR readiness checklist (as all PCTs were
asked to do). The PCT informed the Strategic Health Authority (SHA)that
we were interested and wished to remain informed but did not register as
a fast follower.

The SHA have confirmed Hampshire PCT is only 'potential' and Connecting
for Health have confirmed that this classification does not commit the
PCTs or put them under pressure to become a fast follower.

Currently Hampshire PCT feels there is not enough information about
resources and implication of being a fast follower; with the Hampshire
Health Record (HHR) the PCT feels that our focus should remain on that
project.

Therefore limited work on SCR has been undertaken, with the HHR project
manager attending workshops and remaining informed. No project team has
been set up, therefore there are no related project documents such as
highlight reports. Hampshire PCT has not developed any letters or
communication to patients regarding SCR and has not identified GP
practices to be involved in the scheme.

Given the e-health insider article we will email GP practices to confirm
our commitment to the Hampshire Health Record (previously CDR) and its
relationship with Summary Care Record.
