Dear Sir or Madam,
I would like to know more about how the themes -- which you
allocate to letters you receive about the identity cards scheme --
were created.
I note from a previous request that several of the themes have 0
entries, indicating that some of these themes were created before
any correspondence had been received.
I would like to request:
- All internal correspondence which concerns the creation of these
schemes, or the minutes of any meetings where they were discussed
- All draft correspondence that you may have prepared in readiness
to respond to letters concerning the identity cards scheme which
you receive from the public.

I originally requested this information in March. I then
re-requested it in August, after being told you could not find the
original request.
It is now November and I still have not received a reply.

Dear Identity & Passport Service,
Please pass this on to the person who conducts Freedom of
Information reviews.
Further to my email of November 23rd, for which I have not even
received an acknowledgement, I am writing to request an internal
review of Identity & Passport Service's handling of my FOI request
'Information about the creation of themes for ID cards
correspondence'.
I made the following request on the 25th March this year:

This request was never acknowledged. I followed it up with emails
on the 1st and 14th of July 2009. I finally received an
acknowledgement on the 23rd July, and since then, have received no
communication from you at all. I followed up again with an email on
23rd November this year, to which I have received no response.
By law, you should have replied long ago. Even if you never
received my original request, the subsequent request and its
acknowledgement on 23rd July assured me I would receive a
substantive response by 12th August 2009.
I have received no such response.
Please respond to my original request immediately and provide some
explanation for this delay.
If I do not receive a full and substantive response from you by the
29th January 2010, I will refer this matter to the ICO.
