Individuals bankers failing in their duties to report their actions.

Dear Sir or Madam,
On the governments new website www.realhelpnow.gov.uk
On the page 'credit-crunch-causes-why' a specific claim is made,
vis:
"It was made all the more difficult to fix because sometimes these
investments had been made and bought by individual bankers without
them telling their boards, their auditors or the FSA - so nobody
who should have had a clear picture of what was going on really
did."
Can you tell me if these individual bankers actually had a
responsibility to report their actions to their boards, their
auditors or the FSA?
If they did have such a responsibility, and failed to meet it, can
you tell me how you identified this failure, who these individuals
were, where this responsibility is set out, and what action is
being taken against them for this failure.
If they did not have such a responsibility, then given the
importance you clearly place on it, can you explain why no one in
the regulator had imposed such a responsibility on them?

You have suggested that the treasury would be better placed to
explain why you have drawn attention to a particular point (that
individual bankers did not report their actions to other bodies).
If you cannot explain why you have drawn attention to this
situation, I do not understand how the treasury will know.
Do you not take responsibility for what you publish?
