HPV Vaccine

Dear Sir or Madam,

As a mother of a 15 yr old daughter who has been offered the hpv
vaccine to fight cervical cancer, I would like the statistics for:

% of girls offered vaccine
% of girls that have had it
% of girls/parents that refused consent
% of girls who had side effects
% of girls who had ADVERSE side effects

I hope you can help me with if not all some of these statistics
