how many complainants get comeback requests per year

Dear Sir or Madam,
how many complainants get comeback requests per year, for each year
that you hold information for.
Can you break these down for each office area.
What are the ten most common reasons for requesting review or
comeback ( example, questioning evidence, simply requesting you
review, new evidence, requesting further explanation of your
decision )
How many of the cases each year, that comeback / review is
requested on, lead to the decision being changed modified.
how many question the validity of statements or documents used by
councils in the evidence presented.
What does the ombudsman/investigator do if , after or during an
investigation, it is brought to there attention a document provided
by either side was falsified/edited unreasonably/with held when
presented as evidence ? Please quote relevant acts, guidance,
framework. considered
If you require me to modify any of these questions to make them
easier to answer just email me
Yours faithfully,
George Cant
