What is the usual procedure adopted by by Salford Children's
Services for this kind of scenario, when social workers make these
kind of mistakes ?

I believe that this data requested is of public interest and is data that ought to be published without request being made, in the interests of transparency. As such it is my opiniion that it would be improper to charge for such data. I hope that others may also comment on this point as well.
