Dear Mr Kumar

In response to your request I have received the following details:-

"In early 2004 two doctors who left the list for one month and had to apply
to re-join it in order to continue working as NHS GPs.
However, from later in 2004, and thereafter, the PCT has only required
doctors to apply to re-join the list where they have specified retirement
from both the Practice and the Medical Performers List. If a doctor
specifies that he wishes to retire from the practice but remain on the
Medical Performers List, they are not taken off the list and therefore do
not need to apply to re-join it".

any comments you may have in relation to the service provided.

If you are unhappy with the service you have received in relation to your
