Dear Mr Clark
I must apologise for the protracted delay in responding to your FOI
request. In order that this information could be provided one of our
clinicians has spent a considerable time reviewing casenotes to ensure our
response is as accurate as possible.
As a result of his investigations, of the six cases identified through the
coding mechanism within our data system, only 1 was a true case of
neutropenic sepsis and it was not considered that any alternative
management could have avoided death.
If you are unhappy with the way we have handled your request and wish to
make a complaint or request a review of our decision, you should write to:

If you are not content with the outcome of your complaint, you may apply
