Dear Rob

In response to your request a colleague in the Public Health department has
sent the following details:-

"This level of detail (receiving chemotherapy) is not recorded in the public
health mortality files. Therefore we cannot provide the number of deaths
for the patients had received chemotherapy.

We would be able to query our public health mortality files for patients who
died from febrile neutropenia, although Rob needs to specify the ICD10 code
for the classification of febrile neutropenia, also is he looking at primary
cause of death for this condition or in the subsequent causes, and finally,
we do not have the annual 2008 public health mortality file from ONS. This
file will be made available to Public Health towards the end of the year.
The other source of mortality data we received also comes from ONS on a
monthly basis. This data will also be incomplete due to the data cleansing
at ONS.

Therefore, on this basis I will not be able to provide figures for 2008.
Until, I am provided with the above information, I can not proceed with the
request".

I hope that this response is to your satisfaction and would be grateful for
any comments you may have in relation to the service provided.

If you are unhappy with the service you have received in relation to your
request and wish to make a complaint or request a review of our decision,
you should write to the Complaints Manager at the address below.

If you are not content with the outcome of your complaint, you may apply
directly to the Information Commissioner for a decision. Generally, the ICO
cannot make a decision unless you have exhausted the complaints procedure.
The Information Commissioner can be contacted at: The Information
Commissioner's Office, Wycliffe House, Water Lane, Wilmslow, Cheshire SK9
5AF.

Yours sincerely
