I refer to your email dated 12^th March 2010 making a Freedom of
Information request around ABM University Health Board's formulary.

I can confirm that ABM University Health Board has a Medicines Management
Group (MMG), which is tasked to provide strategic leadership on all
prescribing issues and has a policy-making role on all issues relating to
medicines management. ABM University Health Board operates a joint
formulary between Primary and Secondary Care and this is overseen by the
MMG. Practitioners in both fields are involved in the Medicines Management
Group. Please find attached the Roles and Responsibilities of the
individuals and groups involved in the Merged Entry Process.

You also asked for the names and positions of the members of the D&T
Board. It is not our usual practice to disclose the names of staff other
than those at Executive level; therefore this personal information is
withheld under section 40(2) of the Freedom of Information Act (personal
information). As ABM University Health Board does not have a D&T Board,
please find below the list of members for ABM University Health Board's
Medicines Management Group.

I refer to your email dated 27th April 2010 making a request under the Freedom of Information Act. You asked for copies of the Medicines Management Group minutes. The ABM University Health Board holds this information.

Please find attached minutes relating to the meeting held on 14th January 2010. The next meeting took place on 11th March 2010 and those minutes will be received and approved at the meeting on 3rd June 2010, as the meeting planned for 13th May was cancelled.

You will note that there are redacted sections of the minutes. These all relate to the removal of personal information under the exemption Section 40(2) Personal Information. We have not made such redactions where names are either at Board level or are already within the public domain.
