Dear Sir or Madam,
I am writing to request information about discharge summaries.
Specifically, I would like to ask:
- What standards for the timely issue of discharge summaries has
the Trust agreed with local commissioning bodies?
- What procedures are in place for monitoring the Trust's
compliance with the above standard?
- The number of discharge summaries issued on time by the Trust so
far in the 08/09 financial year and in any other period for which
data are available.
- The number of queries received from GPs or other primary care
staff about the late issue of discharge summaries.

Dear Nottingham University Hospitals NHS Trust,
I sent a Freedom of Information request to you on 30 January 2009
and have still heard nothing from you. By law, under all
circumstances, the Trust should have responded to me by now. This
request is long overdue and I will be making contact with the
Office of the Information Commissioner if I do not hear from you
within the next week.

I have looked through our FOI Log for your original request which you
stated was sent to the Trust on the 30th January 2009 but we have no
record of your original request having been received at the Trust on
this date. Did you receive a confirmation from the Trust of receipt, as
this would be normal practice?
We are very sorry that you have not received a response and are very
concerned that your information request has not been dealt with.
With this in mind, I was wondering if you could possibly send through
your original request again to us for us to action upon receipt.

Dear D Bird,
I am writing in response to your email dated 21st January requesting
information under the Freedom of Information Act 2000, I am pleased to
be able to answer your questions as follows:
Nottingham University Hospitals NHS Trust (NUH) came into being on 1st
April 2006 and assumed responsibility for the services provided by both
the Queen's Medical Centre (QMC) and Nottingham City Hospital NHS Trust
(NCH).
I am writing to request information about discharge summaries.
Specifically, I would like to ask:
1). What standards for the timely issue of discharge summaries has the
Trust agreed with local commissioning bodies?
Within the CQUIN requirements with Commissioners by the 31st March the
Trust is required to provide 75% of discharge summaries electronically
within 24hrs to those GPs who are able and set up to receive them.
2). What procedures are in place for monitoring the Trust's compliance
with the above standard?
IT development processes are in place across the Trust. So far 85% of
specialties have been given training and access to the new Trust
E-discharge template. Roll out to the remaining specialties will be
completed by end of March 2010. Following this all Directorate Managers
will receive a weekly management report on the amount of patients
discharged against discharge summaries created and sent to GPs within 24
hours.
3). The number of discharge summaries issued on time by the Trust so far
in the 08/ 09 financial year and in any other period for which data are
available.
We regret that we are unable to answer this question as we don't have
the monitoring systems in place across the Trust yet as explained in Q2.
4). The number of queries received from GPs or other primary care staff
about the late issue of discharge summaries.
Unfortunately, we do not have any recorded data available to answer this
question.
