Dear Sir or Madam,

In a recent news article[1] it is stated that a passport renewal
was refused to a woman who had changed her name to Mrs Pudsey Bear.

Can you please (a) confirm if the story is true, (b) detail how
many times in each of the last 10 years you have refused to issue a
passport due to the name being frivolous or otherwise likely to
bring to the IPS into disrepute, providing a separate count, where
appropriate, for any cases where after an appeal or similar
procedure the name was later allowed, (c) provide a list of all
names known to have been rejected on such grounds, (d) provide any
handbooks, leaflets, guidelines, memos, emails, or other documents
of any form either for public use or internal, explaining how
decisions are taken as to whether a person's name is deemed to be
acceptable.
