Dear Sir or Madam,

When,where and by whom was the decision not to take notes or
minutes at meetings between BERR and Phorm taken in breach of the
Lord Chancellor's Code of Practice on the Management of Records
which states that "Records of a business activity should be
complete and accurate enough to ... facilitate an audit or
examination of the business by anyone so authorised, [and] protect
the legal and other rights of the authority, its clients and any
other person affected by its actions"?

I am also interested in this information since the recent news of some Lords taking payments to adjust laws going through the Lords.
This is extremely important as it is the only method of proving nothing dubious was planned.
Trust in the work the Lords do would depend on the outcome of such meetings and the notes taken.

Just read the reply to this request for FOI it should be made compulsory that notes are taken to all meetings or no meetings to take place. This does not look good making trust in the government, BERR, ICO or any regulatory body harder.
