Please list all Council's that have ever:

1) rejected either fully or partially a Local Government Ombudsman
['LGO'] report of identifying maladministration and injustice
and/or rejected the LGO recommendations;

2) reneged either fully or partially on a local settlement agreed
between the LGO and the Council;
