Dear Sir/Madam

Thank you for your email of 5 October 2011 where you requested information about applications for Continuing Healthcare funding within the area covered by Hampshire PCT.

The information you requested is provided in the attached document.

The remainder of the information that you requested is exempt under section 12 of the Freedom of Information Act and is therefore not provided.

Section 12 of the Freedom of Information Act 2000 makes provision for public authorities to refuse requests for information where the cost of dealing with them would exceed the appropriate limit, which for health-related departments is set at £450. This represents the estimated cost of one person spending 18 working hours in determining whether the department holds the information, locating, retrieving and extracting the information. NHS Hampshire estimates that it would take in excess of 18 working hours to determine appropriate material and locate, retrieve and extract the information in reference to your request and will not, therefore, progress this element of your enquiry further.

The information supplied to you continues to be protected by copyright. You are free to use it for your own purposes, including for private study and non-commercial research, and for any other purpose authorised by an exception in current copyright law. Documents (except photographs) can also be used in the UK without requiring permission for the purposes of news reporting. Any other reuse, for example, commercial publication, would require the permission of the copyright holder.

If you are dissatisfied with the handling of your request, you have the right to ask for an internal review. Internal review requests should be submitted within two months of the date of receipt of the response to your original letter and should be addressed to:

Freedom of Information reviews, C/O Chief Executive Office, Southampton, Hampshire, Isle of Wight and Portsmouth PCT Cluster, Oakley Road, Southampton, Hampshire SO16 4GX.

If you are not content with the outcome of the internal review, you have the right to apply directly to the Information Commissioner for a decision. Generally, the Information Commissioner's Office cannot make a decision unless you have already exhausted Hampshire Primary Care Trust's internal review procedure. The Information Commissioner can be contacted at: The Information Commissioner's Office, Wycliffe House, Wycliffe Lane, Wilmslow, Cheshire, SK6 5AF.

We would welcome your feedback on how you felt your Freedom of Information request has been managed by NHS Hampshire; your comments are very important and will assist us in monitoring and improving our service. It would be appreciated if you could please complete the attached evaluation form and return it to: [email address] .

Further information about the Freedom of Information Act can be sourced on the Information Commissioner's website: http://www.ico.gov.uk/

If you have any queries about this email, please contact me. Please remember to quote the reference number above in any future communications.

Yours sincerely
