Dear Independent Police Complaints Commission,
On the 12th August the IPCC admitted that:
"we may have verbally led journalists to believe that shots were
exchanged as this was consistent with early information we received
that an officer had been shot and taken to hospital"

Please tell me:
1) Who at the IPCC provided this false information to journalists?
2) What, if any, internal or external investigation has been
conducted or will be conducted into how this false information was
given?
3) What, if any, disciplinary action has been taken against those
responsible for providing false information about the shooting of
Mark Duggan?
4) What changes to IPCC procedures have been made to prevent future
repetition of this type of corruption?
