Complaints against Consultant Psychiatrists (Bootham Park Hospital)

Dear York Hospitals NHS Foundation Trust,

Please provide me with the following information in full:

1. How many complaints have been received regarding the
conduct/attitude/practice/treatment/care of the following
consultant psychiatrists:

Dr Paul Blenkiron
Dr Susan Patricia Shaw
Dr Andrew Elmslie
Dr Bob Adams
Dr Mark Willis
Dr Elizabeth Elsworth

2. What was the nature of each of the complaint(s)?

3. Were the complaint(s) upheld?

4. What was the outcome of the complaint(s), and at what stage was
each complaint resolved?

5. Have any of the above consultant psychiatrists ever undertaken
retraining as the result of a complaint(s) made against them, and
if so what training and when did this take place?

6. Has mental health service provision/delivery ever been
amended/changed due to a complaint(s) being made against any of the
above consultant psychiatrists or any other consultant
psychiatrists?

7. Have any of the above consultant psychiatrists ever raised
concerns regarding patient care/safety in respect of any of their
colleagues?

8. Have any other staff members raised concerns regarding patient
care/safety regarding any of the above consultant psychiatrists?

9. Have any other agencies/organisations raised concerns or filed a
complaint against any of the above consultant psychiatrists?

10. Have any of the above consultant psychiatrists been subjected
to disciplinary procedures, and if so what was the outcome of this?

like to know which organisation holds such records, and how I may
submit a Freedom Of Information Act Request to them to obtain this.
