Dear Mr Killip
As the information which fell within scope of your requests comprised your personal data, we sent it to you by post. The requests you made were in relation to complaints you brought to this Office and we hold your postal address for that reason.
As I said in my last email, if you do not receive our response to your requests, please let us know.
Yours sincerely

Dear Aimee
Thank you for your reply.
I would remind you of the Ministry of Justice's guidance with
regard to Freedom of Information requests, which states, "If a
request is received by email and no postal address is given, the
email address should be treated as the return address."
I have not provided you with a postal address as part of this
Freedom of Information request. Whilst I suspect that you elected
to reply by post for very valid reasons, you accessed my personal
information and sent a reply to the address you had on file for me
without my permission and without checking with me first that the
address was still valid.
Since making the complaint referred to in this Freedom of
Information request I have moved address. By failing to enquire as
to my up-to-date address, you have jeopardised the confidentiality
of the very data you sought to protect.
Yours sincerely

Dear Mr Killip
We would have responded to your information requests via the Whatdotheyknow website had they been Freedom of Information requests. However, your requests were for personal data relating to a complaint you brought to this Office. It would not have been possible for us to process your requests for information without accessing your personal data.
For reasons I'm sure you will appreciate, we would not place someone's personal information on the Whatdotheyknow website; as you will be aware, this website is for Freedom of Information requests and the responses are therefore open to the public at large. It is not an appropriate (or practical) means of providing the personal information requested. Accordingly, we sent our response in writing to the address we held for you following our most recent correspondence with you. We have not been informed by you that your address had changed.
I am sorry that as a result, we have sent our response to an address where you now no longer reside. If you have not received this and would like us to resend the information you requested to a different address, please advise us. We will also update our records. The misdirected correspondence, however, should be returned to us at the return address provided on the envelope.
Yours sincerely

Dear Aimee
Thank you for your reply.
I fully understand the reasons why you elected to send the response
to my Freedom of Information request by post, however, I consider
that it would have been prudent - not to mention polite - to
enquire by email, before posting the response, whether the address
supplied to you well over twelve months ago is still valid.

You may find that Ms Abraham's meeting with the Parliamentary Select Committee gives you some useful quotes on what she states are the operating practice of PHSO - compare with the actuality? and comment. I have further evidence/grounds to ask for a reinvestigation.
on the parliamentlive website. nov 29 2011.
Best of luck!
