Dear United Lincolnshire Hospitals NHS Trust,

I am writing to make a request for all the information to which I
am entitled under the Freedom of Information Act 2000.

I wish to know how many compensation claims or other types of legal
action have been taken against NHS Lincolnshire and how much this
has cost from:

1st January 2005- 31st December 2005,
1st January 2006- 31st December 2006,
1st January 2007- 31st December 2007,
1st January 2008- 31st December 2008,
1st January 2009- 31st December 2009,
1st January 2010- 31st December 2010 and
1st January 2011- 18th October 2011.

If my request is not specific enough, please let me know and I will
do my best to change this.

I would appreciate if you could confirm in writing or by email that
you have received this request.
