Dear Hampshire PCT

Thank you very much for all your previous responses.

I would like to make further requests under the FOI Act.

For the purposes of the Act, the date of your receipt of this
request will be 1st May.

In a previous FOI response, you very kindly supplied me with the
number of bookings recorded to have been made through the Choose &
Book system by each (individual) one of the 21 "Rushmoor & Hart"
Hampshire PCT practices between 1st April 2008 and 31st October
2008.

• In the same way, please could you now kindly supply me with the
number of bookings recorded to have been made per month through the
Choose & Book system by each (individual) one of the 21 "Rushmoor &
Hart" Hampshire PCT practices (i.e. practice codes J82015, J82030,
J82049, J82052, J82061, J82066, J82067, J82099, J82110, J82120,
J82125, J82135, J82138, J82142,J82178, J82181, J82195, J82198,
J82206, J82628 and J82630) between 1st November 2008 and 31st March
2009.

With regards to the HPCT Local Enhanced Service (LES) for Choice
and Booking 2008-2009

• Please could you indicate which of the 21 practices signed up to
component 1 for April 2008 - March 2009

Of those practices that signed up to component 1:

• Please could you indicate the practices that attained "80%
achievement" (or more)

• Please could you indicate the practices that attained "70%
achievement"

• Please could you indicate the practices that attained "60%
achievement"

• Please could you indicate the practices that attained less than
"60% achievement"

I will request information about the remaining practices within
Hampshire PCT at a later date.

As per Section 4a of Part 1 of the FOI Act
(
http://www.opsi.gov.uk/acts/acts2000/ukp...
) I would like the information in question held at the time when my
request is received (draft or otherwise), except that account may
be taken of any amendment or deletion made between now and the
latest time by which the information is to be communicated to me,
being an amendment or deletion that would have been made regardless
of the receipt of my request.

I would be grateful if you would be kind enough to send me the
requested information promptly and in any event not later than the
twentieth working day following the date of receipt of my request -
that is by then end of the working day of 2nd June.

If my request is denied in whole or in part, or specific items
within the responses are withheld from disclosure, then you must
justify all deletions by reference to specific exemptions of the
act, as per Section 17 of the Act

). Where you rely on a qualified exemption to withhold disclosure,
you are obliged to consider the public interest in your decision
and the refusal notice must explain not only which exemption
applies and why, but also the public interest arguments addressed
in reaching the decision.

I wish to receive the information by email; I believe such a
request would be reasonable in these circumstances. As per Section
11 of Part 1 of the FOI Act (

) I would be grateful if you would "so far as reasonably
practicable give effect to that preference"; if you determine that
it is not reasonably practicable to comply with this preference
then you are required by law to notify me of the reasons for your
determination.

The “return address” (to which the information that I have
requested be should be sent ) is the email address that this
particular request originates from (i.e. ending in
'@whatdotheyknow.com').

I would be grateful if you would kindly acknowledge receipt of this
request as recommended by the ICO (“It would be good practice to
acknowledge receipt of requests and to refer to the 20 working day
time limit, so that applicants know their request is being dealt
with”).

I am writing to confirm that Hampshire Primary Care Trust (PCT) has now
completed the search for the information you requested on 1 May 2009 under
the Freedom of Information Act, where you asked for information about
Choose & Book bookings and achievement under LES for the 21 practices
comprising the "Rushmoor and Hart" localities of NHS Hampshire between 1
November 2008 and 31 March 2009.

I am pleased to respond to your individual questions below and, as
previously, the Rushmoor and Hart locality practices have been advised of
this release of information.

Please could you now kindly supply me with the number of bookings recorded
to have been made per month through the Choose & Book system by each
(individual) one of the 21 "Rushmoor & Hart" Hampshire PCT practices (i.e.

Please find attached document detailing Choose and Book bookings made by
the 21 practices comprising the Rushmoor and Hart localities of NHS
Hampshire between 1 November 2008 and 31 March 2009.

I am advised that the attainment levels are not currently held by the PCT
