Dear Hampshire PCT,

I would like to make a request under the FOI Act.

For the purposes of the Act, the date of your receipt of my request
will be 24th November.

Please could you kindly inform me as to the number of bookings
recorded to have been made through the Choose & Book system by
each(individual)one of the 21 Hampshire PCT practices that comprise
the former BLACKWATER VALLEY AND HART PCT (i.e. practice codes
J82015, J82030, J82049, J82052, J82061, J82066, J82067, J82099,
J82110, J82120, J82125, J82135, J82138, J82142,J82178, J82181,
J82195, J82198, J82206, J82628 and J82630) since 1st April 2008.

Please could you also kindly inform me as to the total number of
bookings recorded to have been made, since 1st April 2008, through
the Choose & Book system by the Hampshire PCT practices that
comprise the former following PCTs:

EAST HAMPSHIRE PCT, EASTLEIGH AND TEST VALLEY SOUTH PCT, FAREHAM
AND GOSPORT PCT, MID-HAMPSHIRE PCT, NEW FOREST PCT and NORTH
HAMPSHIRE PCT.

For these 6 former PCTs, I would like the sum total number of
bookings made by all the practices within each former PCT (e.g. how
many bookings were made in total by the 26 practices that made up
the former East Hampshire PCT) - I am not seeking individual
practice numbers for these.

It is my understanding that these figures are not publicly
available.

As per Section 4a of Part 1 of the FOI Act
(
http://www.opsi.gov.uk/acts/acts2000/ukp...
) I would like the information in question held at the time when my
request is received (draft or otherwise), except that account may
be taken of any amendment or deletion made between now and the
latest time by which the information is to be communicated to me,
being an amendment or deletion that would have been made regardless
of the receipt of my request.

I would be grateful if you would be kind enough to send me the
requested information promptly and in any event not later than the
twentieth working day following the date of receipt of my request -
that is by then end of the working day of 22nd December.

If my request is denied in whole or in part, or specific items
within the minutes are withheld from disclosure, then you must
justify all deletions by reference to specific exemptions of the
act, as per Section 17 of the Act

). Where you rely on a qualified exemption to withhold disclosure,
you are obliged to consider the public interest in your decision
and the refusal notice must explain not only which exemption
applies and why, but also the public interest arguments addressed
in reaching the decision.

I wish to receive the information by email; I believe such a
request would be reasonable in these circumstances. As per Section
11 of Part 1 of the FOI Act (

) I would be grateful if you would "so far as reasonably
practicable give effect to that preference"; if you determine that
it is not reasonably practicable to comply with this preference
then you are required by law to notify me of the reasons for your
determination.

The “return address” (to which the information that I have
requested be should be sent ) is the email address that this
particular request originates from (as I am now using a unique
valid email address to track each of my many FOI requests). I refer
you to the Freedom of Information (FOI) Guidance on Processing
Requests ( http://www.dca.gov.uk/foi/guidance/progu... )
which clearly states: "If a request is received by email and,
although no postal address is given, the email address of the
sender is included, then this should be treated as the return
address."

I would be grateful if you would kindly acknowledge receipt of this
request as recommended by the ICO (“It would be good practice to
acknowledge receipt of requests and to refer to the 20 working day
time limit, so that applicants know their request is being dealt

I am writing to confirm that Hampshire Primary Care Trust (PCT) has now
completed the search for the information you requested on 23 November 2008
under the Freedom of Information Act, where you asked for information on
the number of bookings recorded to have been made through the Choose and
Book system since 1 April 2008 by:

o each (individual) one of the 21 Hampshire PCT practices that comprise
the former Blackwater Valley and Hart PCT, and

o the Hampshire PCT predecessor PCTs (as a total of all the practices in

I am pleased to attach information relating to the total number of
bookings recorded to have been made through the Choose and Book system

Hampshire PCT is unable to release the detailed information for each
individual practice within the former Blackwater Valley and Hart PCT at
the present time; the PCT is taking further advice in respect of this part
of your request and anticipates that a final response will be issued to
you by 23 January 2009.
