RE: Certificate of live birth
I refer to my FOI request of 28th of April, requesting some
information about validation of births before registration, as
explained by Mr Hughes, “The Health Service generate notifications
which are received by registrars of births, of all the births that
have occurred in their district”, Is this recorded notification
commonly referred to in some countries as a ‘Certificate of live
birth’ , can you confirm this ?, or whatever name the notification
is given, also of interest are these kept locally within the
district of birth? .

Thank you for your email of 8 December where you ask about National Health
Service birth notifications.
You ask if the notification is commonly referred to in some countries as a
"Certificate of live birth". I am unable to provide comment on the name
given to certain documents or information provided by other countries and
you may wish to direct your question to the relevant authorities in the
countries you are interested in.
You also ask if the NHS birth notifications are kept locally within the
district of birth. I can confirm that some birth notifications are
retained locally within the district of birth as some areas receive a
paper based birth notification via their Primary Care Trust. The
notification of birth is also available electronically to the registrar.

Dear Mel Lee,
Thank you for your response, I do understand that you are unable to
provide comment on the name given to certain documents or
information provided by other countries, what I need to know is
what is the title of these Health Service generated notifications,
and do they have a form identification number(I do not mean serial
numbers) .
You also confirm that some birth notifications are retained locally
within the district of birth as some areas receive a paper based
birth notification via their Primary Care Trust, are the electronic
versions identical to the paper based ones?.
Lastly if I wanted to view my own birth notification, that is
before the registration process took place, if the notification
still exists, would I be able do so, and to whom would I need to
address this request?

I have today received a notification from whatdotheyknow.com
reminding me that my FOI request is long overdue, thus far
exceeding the statutory 20 days response time, this not only
dishonours my request it also dishonours the FOI act itself,
compound dishonours attract penal consequences and form a valid
basis for an internal review, please respond without further delay.

Dear Mel,
Further to your reply of 10th February, I am disappointed that you
are unable provide with the information I seek, however I note you
do advise where I might find it, so in keeping with your
suggestion, I intend in due course to take up the matter with the
NHS, no doubt the information that you have given in your response
of the 14th of February will be of some help with cross referencing
the information I seek from the NHS, so just be absolutely clear
about your reply I need to go over the following, it is based on
the information you have given me.
For and on the record please confirm the following statement, if
you wish to correct of modify this please do so within five working
day of receipt. Otherwise, the whole statement will be deemed to be
accepted by both parties to be true.
In your response of the 14th of January you state that the
information I seek is “not held by the General Register Office”,
local registrars are of course accountable to the GRO, so I take
this the be also applicable to local registrars, and that some
notices of birth are sent electronically to registrars, perhaps by,
e-mail, word or PDF format,it is assumed that any or all of those
apply, if they do not please apply a correction, furthermore that
of the paper based notices, some are retained locally but they do
not appear to have any formal title or basis, no clear
Identification as would normally be the case such as letter
heading, or a official stamp, or form ID of any sort?

responsible for the administration of the law relating to civil
registration and each local authority is responsible for the delivery of
the local registration service within its area.
The General Register Office is unaware of which local authorities hold
or retain paper based NHS birth notifications and again I can only
suggest that you redirect your enquiry to the NHS.
