Dear Blackpool, Fylde and Wyre Hospitals NHS Foundation Trust,
This is a request under the Freedom of Information Act 2000.
Please can you provide me with the number of operations cancelled
in the last six months, until the day this request is dealt with.
Can you break this down specifically into operations cancelled due
to funding cuts/lack of funding, staff illness, double-booked
operations, and any other reasons.
Please can you include any other reasons.
Can this be broken down into departments the operations were under,
and into operations booked within 24 hours and before 24 hours.
Could I have the same data for the six months ending January 12
2010 (or whichever date this request is dealt with).
Could I have the information in electronic format within the time
required by law.
