Dear Fife Council,
Under the Freedom of Information(Scotland)Act 2002 I am requesting
information relating to the above location. The specific period is
from November 2009 to December 2011. The request for information is
the following:
1. Dates and details of inspections undertaken
2. Details of how the inspection was done and reported.
3. Dates and details of identified defects and rectification.
4. Details of inspection frequencies for the above.
5. The specific hierarchy classification of this carriageway
6. Details of repairs undertaken including defect classification
whether patching/gully/frame/cover repair or replacement and
specific location.
7. Dates and details of all gully cleaning undertaken.
8. Details and dates of insurance claims made and the outcome.
9. Details and dates of complaints or enquiries regarding
carriageway defects.

If you are not satisfied with the way we have handled your request, you
have the right to request a review of the decision. Please write to: Chief
Legal Officer, Corporate Services, Fife House, North Street, Glenrothes,
KY7 5LT or e-mail to [1][email address].uk . The law provides for
you to make this request up to 60 workings days from when we received your
enquiry or 40 working days from when you receive this correspondence
whichever is the later date. Where the outcome of the review fails to
resolve the matter to your complete satisfaction you have the right to
apply to the Scottish Information Commissioner for adecision.
Fife Council will consider and seek to resolve any request to the Chief
