Since i am unable to find the information in the public domain,
would you be so kind and to inform me.

Re: the 5th may vote for a new system of vote.

1) Who was responsible for creating the A.V. system

2) who was responsible or authorised the implementation of this
poll(set the ball rolling)

3) At any time, was proportional representation put forward.

3A) if yes, why was it not included.

4) Were there any other systems put forward other than number 3

5) Re Voting slips, how does one go about having the additional box
"I have no confidence in any of the above Candidates", attached to
the ballot paper, either local or national

For the avoidance of any confusion, could you clarify what you mean by question 4: Were there any other systems put forward other than number 3?

Do you mean were there any systems put forward other than the AV system and proportional representation?

In answer to your request to clarify, item number 4.

You are right in your assumption, in that i was asking other than
AV and proportional representation, were there any other forms of
voting systems put forward for consideration.

In order to change the design of ballot papers for UK elections to include an additional box, as requested in Question 5, a number of Acts of Parliament and Regulations would need to be amended. The Cabinet Office of the UK Government is responsible for policy decisions relating to the legal framework for all statutory elections in the UK (except local government elections in Scotland, for which the Scottish Government is responsible).
