Dear Mr Graham,

We would like you to clarify your request. He department has asked if
you could clarify and confirm the comment below for question 5.

Until we hear back from you we will be unable to continue processing
your request. If we do not hear back from you within three months then
your request will be closed.

Thanks
Theodora Brocke-Afful

Clarification of request:
'With reference to question 5, a, we can say whether a patient has been
released or deceased within the hospital but we cannot tell from our
data whether a person has recovered from a condition or not. In addition
one or more of these conditions is incurable so we would not expect a
full recovery.

Please confirm that this is acceptable to let you know who has been
treated and released and those who have died in hospital'.
