Dear Betsi Cadwaladr University Local Health Board,
Could you please supply me with the following data for years
08/09,09/10 and 10/11 for the individual hospitals Bangor,Glan
Clwyd and Wrexham:
1. Number of AF related strokes.
2. Number of AF strokes that were ischaemic.
3. Number of AF strokes that were haemorrhagic.
4. The number known to be in AF on admission.
5. The number that were on antiplatelet on admission.
6. The number that were on anticoagulant on admission.
7. If on warfarin INR on admission.
8. Number of patients that had documented CHADS2 score.
9. Number of patients discharged on an anticoagulant.

Dear Gaynor Gould (BCUHB - Information Governance),
The information given to my questions is confusing, I cannot tell
if Betsi Cadwaladr University Health Board is telling me that they
are unable to identify the differential capacity of patients in
atrial fibrillation who suffer a stroke and patients that suffer a
stroke and are not in atrial fibrillation.
Under your read coding system is the information you have supplied
me with in the tables, patients who have been coded with primary
diagnosis of a stroke but you also know them to be in AF.
Preventing strokes is high on the Welsh assembly governments agenda
but the information that was not supplied with projected costs of
over £4000 is information I would expect the trust to have carried
out as a routine administrative audit. To understand what the
current situation is with patients suffering strokes enables the
trust to see if there health outcomes are improving in both
patients who have strokes and are in AF and patients who have
strokes and are not in AF.
Since 2005 the trust has been aiming for a world class health
service (Designed for Life 2005). Is this a world class performance
when important statistical information such as this is seemingly
unavailable.

Dear Mr Knights
Further to your email below, I can confirm that we are unable to provide
this information as Atrial Fibrillation (AF) cannot be related to the
stroke from the diagnosis coding which is provided on our systems. Our
systems do not allow us to link the 2 together, so if a patient presents
with stroke the AF would be coded as history or active. The systems do
not tell us whether AF is the cause of the stroke, hence the reason for
exempting this information under section 12 of the Freedom of Information
Act 2000 – exceeding the cost limit as it would require an audit of case
notes across the Health Board which would well exceed the 18 hours allowed
under the Act.
