I would like to request a copy of the following information;

1. A copy of the decision of the Inspectorate's decision for this
Appleal.

2. A copy of ALL information pertaining to the application USED in
forming the Inspectorate's decision.

3. A copy of ALL information pertaining to the application NOT USED
in forming the Inspectorate's decision.

4. A copy of ALL information held by the Inspectorate - pertaining
to the application - in any form, notes, photographs, telephone
messages etc....

5. A copy of ALL protocols, policies, procedures relating to the
application.

6. A copy of 'SITE VISIT' protocols, policies,procedures.

7. A copy of the 'Unaccompanied Site Visit' process.

8. A copy of the 'Accompanied Site Visit' process.

9. A copy of the 'Notification of a Site Visit' process.

10. A copy of ALL notes, records, reports compiled during the Site
Visit for this application.

11. A copy of ANY other information relating to this application -
not referred to above.

12. A copy of the details of all parties notified of the decision
of this application.

I will consider your advice as to include ALL documentation
requested, to be sent in hard copy form.

Why can you not provide an electronic version of the information,
which IS currently available in this format? The majority of
application requests, decisions & appeals are usually in electronic
form - thus enabling you to 'Save Paper'.
