Dear Broxtowe Borough Council,

I would like to know the structure of your adult social services
team for commissioning and placement of older persons with
dementia. I would like to have names of team members.

Can you please also indicate minimum, maximum and average weekly
fees for residential care, nursing care, dementia care, dementia
with nursing care, physical disability, acquire brain injury,
challenging behaviour, palliative care,

Also, day care rates for those with dementia and requiring
transport.
