Dear Sheffield City Council,
I would like to know the structure of your adult social services
team for commissioning and placement of older persons with
dementia. I would like to have names of team members.
Can you please also indicate minimum, maximum and average weekly
fees for residential care, nursing care, dementia care, dementia
with nursing care, physical disability, acquire brain injury,
challenging behaviour, palliative care,
Also, day care rates for those with dementia and requiring
transport.

information about are the gross fees the Council pay to care providers OR
the contributions service users have to pay towards these costs.

contribute towards their care costs.
Dementia care will come under the EMI (Elderly Mental Infirm) category in
the above figures for Older People.
There are no specific figures available for Â*acquire brain injuryÂ* or
Â*challenging behaviourÂ*, but the data for the care types of Learning
Disability and Mental Health is provided as it may be of use.
Palliative care is now all provided/paid for by Sheffield Health
Authority.
The structure within the Communities Portfolio which contains adult social
care services is currently changing. The posts of Director of
Commissioning (Miranda Plowden) and Director of Care and Support (Eddie
Sherwood) have both recently been appointed and the structure of these
services may continue to change. The staff within the Older people's
Assessment & Care Management service would be involved in providing
services for older people with dementia. A recent table is attached which
gives an indication as to how these teams are structured within the
Communities Portfolio and a structure chart for the Older People's
Assessment & Care Management service is also attached. Please note that
these are already out of date given recent changes, even though they are
the latest versions we have.
