Thank you for your response, I appreciate that you have taken the
time. However the chart that you provided is not exactly what I am
looking for. If you could please provide me with the teams e.g.
"Reablement team" with the addresses that they are based at, this
would be most helpful. Also if you have Hospital teams, this
information would be very much appreciated.

I write in response to your correspondence which we received on 13^th
October 2011.

As the information you requested is not personal or environmental it has

I have investigated your request and the answers to your questions are as

Q1. Within your Adult Social Care Department, how many teams specifically
deal with elderly care?

A1. Two

 

Q2.  What are these teams called?

A2.  1. The Adult Contact Team, which assesses whether an adult has a
social care need, and then directs them to the relevant reablement/support
service. 

2. Housing Solutions (aka Housing Aid) provides help to people who are
homeless or who may become homeless. 

 

Q3. Address and contact details for each team.

A3. The Adult Contact Team contact details are: Mary Potter Centre,
Gregory Boulevard, Nottingham, NG7 5HY or telephone 0115 8838460. Housing
Solutions contact details are 135 Lower Parliament Street, Nottingham, NG1
1EE or telephone:0115 9153000.
