responsible for elderly care. Your request has been carefully considered

Working age adults with mental health problems are referred to the 2gether
Trust.

 

Adults with a learning disability (18+ but also includes older people 65+
unless it is decided that at time of assessment their age related needs
outweigh any learning disability needs) are referred to the Countywide
Learning Disabilities Team. Sensory impaired service users are managed by
area with the sensory team involvement.
