Dear Ms Moore
Regarding your request for information about whether birth parents
automatically get notified if an adoptive placement breaks down:

Birth parents do not automatically get notified if this occurs.

The adoptive parents have the responsibility  and the discretion to make
decisions in the best interests of their child as to whether there will be
contact with birth parents and what form this contact will take,   Often
it will be in the form of an annual letter or photographs.

The social work department have no authority to share the child's
information  with the birth parents once the adoption has taken place.

The birth parents lose all parental rights and responsibilities once their
birth children become adopted. The rights and responsibilities are
conferred onto the new adoptive parent/s who are then the  ones who make
decisions about what information is shared about their child's
circumstances.

I hope this clarifies your query
Regards
Rosemary Morgan
